Position Description:


They will be generally see about 5-8 patients per day. They may be in a home or a facility and will be required to travel onsite to care for the patient. They will assist with bathing, dressing, hygiene for patients; Lunches, dinners, and companionship. They will be responsible for documenting the visit. The documents are paper base charting. They are required to turn this in within 24 hrs of the visit. Case load of 12-16 patients; Travel is 30+ miles in between patient visits.

Skills / Requirements

Requirements:

• Certified C.N.A. license in the State of Missouri


• 1+ year Home Health and / or Hospice experience


• 2+ years clinical experience


• Reliable transportation, valid DL License with no restrictions, and insurance is required


• HS Diploma or GED is required

Assets:

• Experience with Senior Population a plus
